Beneficiary Form – The form is for the participant to choose the beneficiary of the plan assets in the event of his/her death.
Change of Name Address Form – This form is for the participant or the employer to submit a name or address change for a participant.
Request for Hardship Distribution Form – This form is for the participant to request a hardship distribution from the plan. A plan may or may not allow for these types of distributions. In addition, the plan may restrict these distributions to a certain source of money. All loans from the plan must be exhausted before a hardship distribution will be granted. AMI will request documentation to verify the distribution necessity and amount. If the Hardship Distribution can be granted, AMI will mail to the participant all of the necessary documents to be reviewed and/or signed.
Request for In-Service Distribution Form – This form is for the participant to request a distribution from the plan while they are still working, providing that the plan permits these types of distributions and the participant has met the minimum age requirement for this type of distribution. If the In-Service Distribution can be granted, AMI will mail to the participant all of the necessary documents to be reviewed and/or signed.
Request for Loan Form – This form is for the participant to request a loan from his/her plan assets. The plan must allow for loans and the participant must be vested in at least $2,000 to obtain the loan. AMI will apply all of the loan limit rules to determine if a loan can be granted. If the loan can be granted, AMI will mail to the participant all of the necessary documents to be reviewed and/or signed.
Salary Reduction Agreement – This form is for the participant to request a change in the amount this is being withheld from his/her paycheck. This form only applies to traditional 401(k) plans. If the participant is enrolled in an “Automatic Enrollment” plan, the participant will need to log into “Benefit Information” and change his/her salary reduction amount electronically.
Request for Eligibility Determination – This form is for the employer to request whether or not an employee is eligible for the plan. It is most often utilized when an employee has left employment and returned to work.
Employer Reporting of Term – This form is for the employer to report to AMI that a participant has terminated. AMI will mail to the participant all of the necessary documents to be reviewed and/or signed. AMI will also mail to the employer the form to be autographed to instruct AMI to proceed with the distribution as specified by the participant.
Nondiscrimination Testing Questionnaire (Standard) or Nondiscrimination Testing Questionaire (PEO) – This form is for the employer to complete at the end of each plan year so that AMI can complete the necessary nondiscrimination tests for the plan.
Ownership/Officer Information (Corporate), Ownership/Officer Information (Partnership) or Ownership/Officer Information (Sole Proprietor) – This form is for the employer to complete at the end of each plan year, providing AMI ownership/officer information that is needed to accurately complete non-discrimination tests for the plan.
